Bad vibration

My 92 d21 4x4 Hardbody has a bad vibration when i pull away from a dead stop i have already replaced u-joints and center support bearing. any ideas?

Drum brakes? If you have drum brakes then they could be warped... possibly. does it shake at 45 miles an hour?
