Default 96 Altima engine cuts out at 4k rpm when hot

Hello everyone. I own a '96 Altima with 193,000 miles on it. When the engine is cold, everything is fine. Once the engine warms up, if I accelerate, causing the engine to shift down and rev the motor to around 4k rpm, the engine will cut out. Often times the engine will completely shut down. Sometimes the car has to sit for awhile before it starts back up. I hooked up an OBD II scanner to it and got code P0340 "cam position sensor(phase) fault." I replaced the cam position sensor (set me back $57). The car still does the same thing. Any ideas as to what might be causing the problem? Thanks in advance.

where did you find the replacement sensor?
was it difficult to replace?
